Northern Health are seeking applications from motivated and suitably experienced Effectiveness and Outcomes Managers to join our Clinical Leadership, Effectiveness and Outcomes (CLEO) team for Fixed Term, 2 years.
Northern Health is the major provider of acute, maternity, sub-acute, mental health, specialist, community and home-based services in Melbourne’s rapidly growing outer north.
Services are provided through our five main campuses :
Northern Hospital Epping, Broadmeadows Hospital, Bundoora Centre, Craigieburn Centre, and Kilmore District Hospital. In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.
About the Role :
This is an exciting opportunity for a motivated and adaptable professional to join Northern Health’s Clinical Leadership, Effectiveness and Outcomes (CLEO) Team.
The Effectiveness and Outcomes (E&O) Manager plays a central role in leading programs that improve health outcomes, strengthen clinical effectiveness, and embed value-based, person-centred care across the organisation.
The role provides project management leadership across programs involving digital health technologies, remote patient monitoring, and model of care redesign. Working closely with clinicians, consumers, and digital health partners, you will co-create care models that are sustainable, person-centred, and measurable in their impact.
A key focus will be supporting the implementation of Safer Care Victoria’s Assisted Self-Management (ASM) Program, a state-wide initiative empowering people living with chronic conditions such as COPD, heart failure, and diabetes to take an active role in managing their health through self-management education, medication support, and risk-stratified remote monitoring.
The position also contributes to Northern Health’s broader Outcomes for Impact strategy, helping to build organisational capability in Value-Based Healthcare, outcomes measurement, and digital transformation.
