Please note that this is Permanent Part Time Position(s), based in Epping
Reporting to the Director - CLEO, the Effectiveness and Outcomes Manager is responsible for the redesign of clinical pathways and the implementation of remote patient monitoring and digital technology for Northern Health to enable to collection and improvement of patient outcomes. This includes ensuring that the pathways or remote patient monitoring are delivered in agreed timeframes and in accordance with the business objectives and key results. This role is also responsible for facilitating lived experience groups with consumers and education in Value Based Healthcare and digital technology. The Manager role facilitates collaboration between all stakeholder involved in the delivery of digital health initiatives and supports the implementation of new technologies aimed at achieving better outcomes for our community.
