SustainHealth is seeking an experienced Primary Health Registered Nurse to join the dedicated team in Broken Hill, NSW delivering high-quality, culturally sensitive healthcare to Aboriginal communities in far western NSW.

Salary : Up to $106,000 p.a. + Super (depending on qualifications & experience)

About the Role Reporting to the Clinic and Community Coordinator, you’ll play a vital role in providing patient centered care across both clinic and community settings. Your responsibilities will include :
- Coordinating and assisting with treatments and care activities in a general practice clinic.
- Conducting health assessments, wound care, chronic disease management, vaccinations, procedural support, and triage.
- Delivering services in the community, including home visits, domiciliary care, and follow-up for Aboriginal people aged 15+ living with chronic conditions.
- Working collaboratively with Aboriginal Health Practitioners and trainees using a case management model.
- Building lasting, trusting relationships with patients, families, and GPs.

Mandatory Credentials
- Blood collection / phlebotomy (vacutainer)

- Nationally recognized vaccination certification
- Basic Life Support (CPR) minimum

About You
- Current AHPRA registration as a Registered Nurse
- Proven experience in primary health, community, or general practice settings
- Strong clinical skills in assessment, chronic disease care, and preventative health
- A commitment to culturally safe practice and improving Aboriginal health outcomes
