About the Role

Reporting to the Clinic and Community Coordinator, you will provide  patient-centred care across both clinic and community settings , working closely with Aboriginal Health Practitioners and trainees.

Key Responsibilities :

• Coordinating and assisting with treatments in a  general practice clinic
• Performing  health assessments, wound care, chronic disease management, vaccinations, procedural support, and triage
• Delivering  community-based services , including home visits and follow-up for Aboriginal people aged 15+ with chronic conditions
• Collaborating with GPs and healthcare teams to ensure holistic, culturally safe care
• Building  trusting and lasting relationships  with patients and families

Mandatory Credentials

• Blood collection / phlebotomy (vacutainer) certificate
• Electrocardiogram
• Must have vaccination certification (nationally recognised)
• Basic Life Support (CPR)
• Computer literacy (Medical Director & digital comms platforms)

About You

• Current  AHPRA registration as a Registered Nurse
• Must have full working right in Australia - no sponsorship is available
• Experience in  primary health, community, or general practice nursing
• Strong clinical skills in assessment, chronic disease, and preventative health
• Committed to  culturally safe practice  and improving Aboriginal health outcomes
