Key Responsibilities :
- Obtain sufficient patient information necessary to function effectively in the assigned home healthcare setting
- Provide professional nursing care using all elements of the nursing process
- Assess and evaluate patient status by :
- Writing and initiating individualized care plans
- Regularly re-evaluating patient needs and response to treatment
- Revising the care plan as necessary
- Implement physician-prescribed orders for medications and treatments
- Apply appropriate preventive, therapeutic, and rehabilitative nursing techniques
- Maintain effective communication regarding the patient's condition and care with the healthcare team and family
- Assist patients with activities of daily living (ADLs) to support recovery and quality of life
- Perform only those services and procedures within the scope of your training and experience
- Accept and perform specialized procedures such as chemo port needle insertion, PICC line dressing, catheterization, Ryles tube insertion, etc., only when properly trained, competent, and authorized
