Job Summary :

We are seeking an experienced and detail-oriented Medical Coder – HCC (Hierarchical Condition Category) to join our dynamic healthcare team. The ideal candidate will have a strong background in HCC coding, with at least 3 years of relevant experience, and must be comfortable working night shifts . The role involves reviewing medical records and assigning accurate diagnosis codes to ensure compliance with CMS risk adjustment guidelines.

Key Responsibilities :

• Review and analyze medical records to assign accurate HCC / ICD-10-CM diagnosis codes
• Ensure coding is in compliance with all CMS and organizational guidelines
• Abstract and validate patient data to capture all applicable chronic conditions
• Collaborate with providers and documentation specialists to clarify diagnoses
• Meet coding productivity and quality benchmarks
• Stay updated on industry regulations, payer policies, and coding changes
• Maintain strict patient confidentiality and adhere to HIPAA regulations
