About the Role :

• This is a multi-classified position with possibility of appointment at either HPO3, P3 or RN2. Please review 02OEZ if you are a HPO3 or P3 interested in this position. There is only one position available.

Join the Paediatric Liaison and Navigation Service (PLaNS) as a Paediatric Care Navigator and make a real impact in the lives of children with complex health needs across the ACT. This dynamic full-time role puts you at the heart of a multidisciplinary team, where you’ll coordinate care, connect families with vital services, and lead improvements in health outcomes through innovative, family-centred approaches. You’ll work across primary, secondary, and tertiary care settings, build strong relationships with interstate and local providers, and help children stay well in their communities.
