Role & responsibilities
- CCN Patient visits Once weekly and New patients within 24hours of onboarding
- Family Counselling : Current treatment, prognosis and providing empathetic and compassionate support.
- Primary Consultant update Once weekly and SOS (whenever required)
- Interdisciplinary Collaboration : Consulting with multiple specialties ensuring comprehensive patient care and treatment plans
- Guiding and Mentoring of nursing staff : Clearing all doubts / misconceptions at point of care enhancing their knowledge, thereby motivating for excellent patient care and professional growth
- Shadow auditing - During weekly home visits, auditing nursing notes, medication chart and grooming of the nursing staff
- Ensuring Clinical Excellence - Checking medication chart daily, video call rounds, coordinating with ICU clinical manager.
- Death and Discharge summary.
- Single time visits - On request / demand supporting Doctor@Home
- Clinical Escalation Management : Issues which could not be resolved at point of contact are raised to superiors as per the protocol
