About the Role
We’re seeking a Home Care Coordinator who will be the key link between clients, families, and caregivers. You’ll manage care plans, schedules, and service delivery to ensure every client receives timely, personalized, and high-quality care at home.
Key Responsibilities
- Coordinate and monitor home care services for clients.
- Design and update care plans based on client needs.
- Schedule and assign caregivers or home health aides.
- Communicate regularly with clients, families, and healthcare teams.
- Maintain accurate care documentation and reports.
- Resolve day-to-day service issues or emergencies efficiently.
- Ensure compliance with internal and healthcare quality standards.
Qualifications & Skills
