An exciting opportunity has arisen for Team Lead to join the NHS Borders Hospital at Home team. We are looking to recruit an enthusiastic, motivated Advanced Nurse Practitioner (or working towards) with excellent communication and clinical practice skills to work within the community setting.

The Team Lead will have operational management and professional leadership for staff within the Hospital at Home team. Responsible and accountable for the delivery of skilled care to the patients referred to the team. The Hospital at Home Service will provide advanced clinical care to patients from Acute Services who can be safely managed at home or where hospital admission can be prevented for patients in the Community.

The post holder will undertake the role of a Team Lead, utilising high levels of decision making and clinical judgment, with the skill and knowledge developed with the Advance Nurse Practitioner qualifications. The post holder will function as an independent autonomous practitioner by virtue of in-depth knowledge, expertise, proficiency and experience and will lead and enable the team to provide a high quality service.

Currently the service is 7 days, 0800-1800 daily. The post holder for the Team Lead post would work Monday – Friday with weekend working.
