Key Responsibilities

• Deliver Tele-Coaching Sessions : Provide structured life and / or wellness coaching sessions via video or phone calls to support adults in setting goals, building confidence, and encouraging action to achieve meaningful and sustainable change.
• Collaborate with Support Teams : Work closely with our dedicated provider support team, licensed clinical care navigation team, and administrative care support team whenever needed.
• Provide Culturally Competent Care : Uphold our values of diversity and inclusion by providing empathetic and culturally sensitive care to clients from diverse backgrounds.
