About the Role

We’re looking for a compassionate and skilled Elder Care Coordinator with a paramedic or clinical background to oversee personalized care for senior clients. You’ll serve as the vital connection between elderly clients, their families, caregivers, and healthcare professionals — ensuring that every senior receives safe, timely, and dignified care at home.

Your role will combine clinical insight , care coordination , and empathy to help elders live healthier, more independent lives with the right medical and emotional support.

Key Responsibilities

• Coordinate and monitor elder care services for clients at home or in assisted settings.
• Assess and document seniors’ health needs , vital signs, and risk factors in collaboration with doctors and nursing teams.
• Design, implement, and regularly update personalized elder care plans based on medical conditions and daily living needs.
• Guide and support caregivers on proper elder care practices, medication adherence, and emergency protocols.
• Respond to medical or wellness emergencies promptly using paramedic skills until advanced help arrives, if needed.
• Communicate regularly with families, physicians, and care teams to ensure continuity of care and emotional well-being.
• Maintain accurate care documentation, progress notes, and health reports.
• Ensure compliance with clinical, safety, and quality standards in elder care delivery.
