Job description
  • Assess client needs and develop care/support plans
  • Coordinate with professionals (doctors, therapists, social workers, educators)
  • Schedule appointments and manage service delivery
  • Act as a link between clients, families, and service providers
  • Monitor progress and update case records.
  • Ensure timely and efficient service coordination
  • Maintain documentation and reports
  • Help clients access community resources and support services

    • Requirements

  • Knowledge of case management and care coordination
  • Strong organizational and time-management skills
  • Good communication and interpersonal skills
  • Problem-solving and decision-making ability
  • Empathy and ability to handle sensitive situations
  • Basic computer and documentation skills
  • Relevant work experience (preferred)
