• Lead, mentor, and train a multidisciplinary team of health professionals including Dieticians, Care Managers, and Mental Wellness experts.
• Conduct regular performance reviews and provide continuous professional development opportunities.
• Foster a collaborative and dynamic team environment.
• Supervise patient assessments, care plans, and rehabilitation progress.
• Monitor patient outcomes and implement strategies to enhance efficacy.
• Drive patient engagement and satisfaction to increase referrals and renewals.
• Leverage data and technology to enhance patient care and operational efficiency.
• Collaborate with technology teams to develop new tools and solutions for cardiac rehabilitation.
