Job Summary

The Home Care Nurse will be responsible for assessing, planning, implementing, and evaluating patient care, while working collaboratively with the healthcare team to ensure the well-being of clients in need of home care support.

Key Responsibilities & Accountabilities

• Develop and evaluate nursing care plans
• Monitor vital signs and assess holistic patient needs
• Provide client and family education
• Collaborate with physicians and healthcare team members
• Administer medications and maintain medication safety
• Document nursing care and procedures accurately
• Implement and maintain infection control
• Perform nursing procedures as necessary
• Attend training and seminars as required
• Other tasks as assigned
