Position Overview

The HL7 Interface Engineer is responsible for the comprehensive management of healthcare interoperability solutions focusing on laboratory diagnostics data exchange. This specialist will design, develop, implement, monitor, and maintain HL7 interfaces between our Harvest Laboratory Information System (LIS) and various external healthcare systems including healthy systems, physician practice EHRs, reference laboratories, and insurance / billing platforms.

Key ResponsibilitiesLIS & Interface Administration

  • Develop and maintain bidirectional HL7 v2.x interfaces (primarily v2.5.1) between Harvest LIS and diverse EMR / EHR systems (Epic, Cerner, Meditech, Allscripts, athenahealth, eClinicalWorks)
  • Configure and map complex laboratory order codes (LOINC, CPT, proprietary codes) across systems to ensure accurate test ordering and resulting
  • Implement customized interface solutions for specialized workflows, including reflexive testing, microbiology culture / sensitivity reporting, and genetic / molecular testing results
  • Monitor interface message queues and resolve transmission errors within established SLAs (
  • Manage complex interface routing logic based on message content (test type, physician, facility, insurance)
  • Configure and maintain LIS system settings including :

Practice / Client configurations with customized requisition forms and result delivery preferences

Location hierarchies and specimen collection sites

User security profiles and role-based access control

Order choice dictionaries with associated billing codes and reference ranges

Mirth Connect & Secure Data Transfer Management

  • Serve as primary administrator for Mirth Connect Enterprise interface engine (version 4.1+)
  • Design, develop, and deploy new Mirth channels using JavaScript for transformation logic
  • Optimize existing Mirth channels for performance, scalability, and error recovery
  • Configure custom channel destinations based on message content and business rules
  • Implement advanced message transformation between formats (HL7 v2.x, FHIR, XML, JSON, CSV, database)
  • Develop specialized data handling for non-standard laboratory data (genetics reports, PDF documents)
  • Create and maintain comprehensive channel documentation and configuration management
  • Implement high-availability configuration with active / passive Mirth nodes and automatic failover
  • Configure secure data transfer protocols :
  • Site-to-site VPN connections with healthcare partners
  • SFTP / FTPS servers with PGP encryption for batch transfers
  • TLS-secured MLLP connections for real-time messaging
  • REST and SOAP web service endpoints with OAuth2 authentication
  • Mapper Script Development & Vendor Integration

  • Develop and optimize JavaScript mapper scripts for Harvest LIS and Mirth Connect
  • Create custom mapper functions for specialized data transformation requirements :
  • Complex specimen handling instructions
  • Anatomic pathology and cytology reporting
  • Results with interpretive comments and abnormal flags
  • Reference range management across different patient populations
  • Debug and optimize complex mapper logic to ensure data integrity
  • Develop quality assurance processes for interface deployments including test plans, validation protocols, and acceptance criteria
  • Work directly with EMR vendors to configure and troubleshoot integration points
  • Coordinate with reference laboratories for outbound testing interfaces (LabCorp, Quest, Mayo Medical Laboratories)
  • Manage interface specifications documentation and maintain accurate interface inventory
  • Monitoring, Maintenance & Support

  • Implement proactive monitoring of all interface channels with automated alerting
  • Develop custom dashboards for real-time interface performance metrics
  • Provide 24 / 7 on-call support for critical interface issues (rotation schedule)
  • Conduct regular interface audits to ensure data integrity and compliance
  • Participate in change management processes for all interface modifications
  • Develop and maintain disaster recovery procedures for all interface systems
  • Create and maintain comprehensive documentation for all interfaces
  • Technical RequirementsHL7 Interface Requirements

    Message Types : Expertise implementing and troubleshooting specific laboratory message pairs :

  • ORM / ORU (Orders & Results) for routine laboratory testing
  • OML / ORL for microbiology and specialized testing
  • ADT (A01, A04, A08) for patient demographics management
  • DFT for laboratory billing transactions
  • MDM for text reports and PDF document exchange
  • Segment Expertise : Advanced knowledge of laboratory-specific segments :

  • OBR (Observation Request) configuration for test ordering
  • OBX (Observation Result) for discrete and formatted text results
  • SPM (Specimen) for complex specimen handling requirements
  • NTE for comments and interpretive reporting
  • PV1 / PV2 for patient encounter context
  • Implementation Experience :

  • Z-segment development for laboratory-specific data requirements
  • Implementation of LOINC, SNOMED, and CPT coding in interfaces
  • Management of abnormal flags, reference ranges, and critical value alerts
  • Complex specimen identification and handling across systems
  • Results authentication and signature requirements
  • Mirth Connect Requirements

  • Minimum 3+ years experience with Mirth Connect interface engine (v3.8 or higher)
  • Advanced JavaScript programming for custom channel logic and transformations
  • Experience with Mirth Connect Enterprise features :
  • Channel groups and dependencies
  • Code templates and libraries
  • High-availability configuration
  • Enhanced monitoring and alerting
  • Ability to develop custom Mirth extensions as needed
  • Laboratory Information System Experience

  • Prior experience with Harvest LIS preferred (minimum 2 years)
  • Understanding of laboratory workflow including :
  • Order entry and specimen accessioning
  • Results review and release processes
  • Quality control and regulatory requirements
  • Billing and coding for laboratory services
  • Experience with laboratory instrumentation interfaces (Roche, Abbott, Beckman Coulter, Siemens)
  • Additional Technical Skills

  • Network protocol knowledge (TCP / IP, HTTP / S, SSH, TLS)
  • XML and JSON data structure expertise
  • REST API development and integration
  • System monitoring tools (Prometheus, Grafana)
  • Version control systems (Git)
  • Experience with secure healthcare data exchange protocols
  • Qualifications

  • 5+ years experience in healthcare interface development
  • HL7 certification preferred (HL7 v2.x Specialist or equivalent)
  • Mirth Connect certification preferred
  • Experience with laboratory workflows and LIS systems
  • Strong problem-solving and analytical thinking skills
  • Excellent communication skills to work with technical and non-technical stakeholders
