Job Summary :

We are seeking an experienced and detail-oriented Medical Coder – HCC (Hierarchical Condition Category) to join our dynamic healthcare team. The ideal candidate will have a strong background in HCC coding, with at least 3 years of relevant experience, and must be comfortable working night shifts. The role involves reviewing medical records and assigning accurate diagnosis codes to ensure compliance with CMS risk adjustment guidelines.

Key Responsibilities :

• Review and analyze medical records to assign accurate HCC / ICD-10-CM diagnosis codes
• Ensure coding is in compliance with all CMS and organizational guidelines
• Abstract and validate patient data to capture all applicable chronic conditions
• Collaborate with providers and documentation specialists to clarify diagnoses
• Meet coding productivity and quality benchmarks
• Stay updated on industry regulations, payer policies, and coding changes
• Maintain strict patient confidentiality and adhere to HIPAA regulations
