About the Role

We’re looking for a compassionate and skilled

Elder Care Coordinator

with a

paramedic or clinical background

to oversee personalized care for senior clients. You’ll serve as the vital connection between elderly clients, their families, caregivers, and healthcare professionals — ensuring that every senior receives safe, timely, and dignified care at home.

Your role will combine

clinical insight ,

care coordination , and

empathy

to help elders live healthier, more independent lives with the right medical and emotional support.

Key Responsibilities

Coordinate and monitor

elder care services

for clients at home or in assisted settings.

Assess and document seniors’ health needs , vital signs, and risk factors in collaboration with doctors and nursing teams.

Design, implement, and regularly update

personalized elder care plans

based on medical conditions and daily living needs.

Guide and support caregivers

on proper elder care practices, medication adherence, and emergency protocols.

Respond to

medical or wellness emergencies

promptly using paramedic skills until advanced help arrives, if needed.

Communicate regularly with

families, physicians, and care teams

to ensure continuity of care and emotional well-being.

Maintain accurate

care documentation, progress notes, and health reports.

Ensure compliance with

clinical, safety, and quality standards

in elder care delivery.

Qualifications & Skills

Certified Paramedic

/ Diploma or Graduate degree in

Paramedical Sciences, Nursing, or Healthcare Management .

1–3 years of experience

in home healthcare, elder care, or patient coordination roles.

Strong understanding of

geriatric health needs, chronic care management , and

basic medical procedures .

Excellent

communication, empathy, and interpersonal

skills — able to engage effectively with seniors and families.

Organized and proactive with strong

problem-solving and multitasking

abilities.

Comfortable using

digital health platforms

and scheduling tools for care management.
